Athletic Training Clearance Information

Student-athletes must complete the appropriate athletic training clearance requirements before participation. Please select the section below that matches your status for the upcoming academic year.

Returning Student-Athletes

For student-athletes that played a sport at Keuka College in 2025-26.

Health Portal forms are due by June 1.

For student-athletes that did not play a sport at Keuka College in 2025-26.

Health Portal forms are due by July 15.

Intro Letter for New Student-Athletes Keuka Student Health Portal Instructions Physical Form for 2026-27 Academic Year

All sports must have their physical dated after Feb. 24, 2026.
Esports does not require a physical.
