New Student-Athlete Forms

(For Student-Athletes that DID NOT play a sport at Keuka College in 2025-26)

  1. Intro Letter for New Student-Athletes
  2. Keuka Student Health Portal Instructions (Due by July 15)
  3. A Physical must be completed by your Physician (per NCAA) - Physical Form (Updated for 2026-27 academic year)
    1. All sports must have their physical dated after Feb. 24, 2026
    2. Esports does not require a physical
